You don’t need a green card to go to medical school in the US, just a student visa and the money to pay the tuition. Once you’re qualified as a doctor, if you chose to try to work in the US you will have to have an employer apply for your work visa.

And to David, "no shortage of medical doctors in the US"…WHAT ARE YOU SMOKING?! We have a HUGE shortage of primary care providers in this country. Ditto nurses and technicians. You should pay more attention to what’s going on around you.
